Can anyone tell me how i can obtain my child's shot records myself, without going through the doctor's office?

i was wondering how i might be able to obtain my child's immunization records without going through the doctor's office. i'm not too sure that i'm really happy with this physician, and i would like my child(ren) to be seen y someone else. i have copies of older shot records, but i don't recall her office giving me his recent one. is there an agency i can call or go online and request myself? we live in louisiana, (st tammany parish). thank you. does anyone know of a pediatric doctor that sees adolescents?. my kids are 17, 16, and 13.
